Support Coordination

Our support coordinators are here to help you unpack your plan and ensure that you are utilising it to the best of your abilities. We can connect you to supports you need, advise on helpful resources and assist you with all of your your plan reviews.

• Our support coordinators have worked in the disability sector and are aware of the legislative requirements to ensure that the specific recommendations that are implemented for you align to your plan and your individual goals. Our support coordinators are there to work for you and help you reach your goals. We will listen to your needs and ensure that your wishes are put into action. Your choice and control are at the forefront of your plan.

• TnC Care work closely with numerous allied health professions to ensure that you are getting the most from your NDIS plan. We understand that one size doesn’t fit all, which is why we are aligned with different allied health providers to ensure that we find one that suits you.

• Our support coordinators can empower you to organise your supports on your own. They work with you to develop the knowledge, skills and the confidence you need to set and achieve your goals in the future.

121 – Interpret and Translate

Participants from culturally and linguistically diverse (CALD) backgrounds may need interpreting assistance to communicate with providers. TnC Care offers interpretation services in various languages. Our interpreters maintain strict standards of professionalism and possess exceptional linguistic and interpersonal skills to help facilitate interactions between families, government bodies and allied health professionals.

According to the Federal Government’s Multicultural Language Service Guidelines: On-site interpreters should be used in instances where communication involves::

  • Sensitive, complex or lengthy discussions
  • Documents and consent forms (i.e. when an interpreter provides a sight translation of a written document)
  • Group meetings or interviews planned in advance Or
  • Where for practical reasons, telephone interpreting is not appropriate; for example, where there is heavy reliance on mobile phones.

136 – Group Centre Activities

At TnC Care we aim to ensure that all participants enjoy a wide range of social experiences and activities including sports, arts and craft, music and special activities such as trivia, discos and day trips. We can also help you to develop your skills with classes in cooking, job skills and money handling.

Our aim is to provide all participants with a safe, welcoming and fun place to share experiences and build friendships. We can tailor activities to suit your needs and to ensure that each activity is assisting you to improve your skills and abilities along the way.

101 – Accommodation and Tenancy

Under the NDIS, Short Term Accommodation and Assistance (STA) replaces what used to be called ‘respite’ for people living with a disability. Short Term Accommodation and Assistance can be a short stay in a property with other people or additional support in your own home. Our Short-Term Accommodation services can give you the chance to meet new people and try new activities or have a get away with a support worker to unwind and give you a much-needed break. You may choose to attend a variety of activities such as playing sports, going to the movies or the theatre, going out for a meal, going to the beach or attending local festivals and attractions. We organise everything for you, from the accommodation, meals, attractions and support workers.

116 – Innovative Community Participation

TnC Care provide individualised one on one and group activities designed to increase skills, independence and confidence in a caring, fun and social environment. Innovative community participation can lead to improved wellbeing outcomes for people living with disability and their carers. The community as a whole benefit from inclusive arrangements.

108 – Assist Travel and Transport

TnC Care can provide transportation services to enable participation in the community, social, economic and daily life activities. This includes, but is not limited to, taxis and specialist transport arrangements. Travel and Transport is provided when the participant is unable to access alternative forms of transport as a result of their disability. There are three levels of transportation funding under the NDIS and we are able to assist you to understand how your funding can be utilised over the course of your plan.

107 – Daily Personal Activities

TnC Care have support workers that can assist you from a few hours per day to full 24-hour support. Tnc Care support workers provide a holistic person-centered approach in a variety of activities including but not limited to:

  • Personal Care and daily living support
  • Budgeting, written communication, paying bills
  • De-cluttering
  • Food preparation and cooking at home or meal delivery
  • Shopping
  • Home and Garden Maintenance
  • Social and Community Participation
  • Transport to activities and appointments
  • Interpreter and Translation Services
  • Short Term Accommodation/Respite

120 – Household Tasks

TnC Care offers assistance with domestic chores, including assistance with cleaning, dishwashing, clothes washing, fresh linen and ironing, in order to maintain a safe, secure and healthy environment for our participants.

Services Available

Essential cleaning is offered across all areas that are regularly used by our participants within their own home.

These include:

  • Bathrooms
  • Toilets
  • Kitchens
  • Laundries
  • Living areas and bedrooms.

Support workers undertake tasks such as cleaning stoves tops, wiping down bench tops and inside fridges as well as vacuuming or mopping floors, dusting, dishwashing, changing bed linen, washing, drying and ironing.
