
About TnC CARE
Town and Country Aged Care PTY LTD trading as TnC Care has been a Registered NDIS provider since 2017 but has been involved in the disability sector for more than 10 years through community participation, education and volunteer work.
TnC Care offers individual supports and services in a holistic approach for people living with disabilities throughout Melbourne and regional Victoria.
TnC Care was established by our dedicated Directors who have a genuine connection to the disability sector. We strive to ensure that some of the most vulnerable people in society are provided with safe, high quality care, and are treated with dignity and respect by our staff at all times. We strongly believe that our clients’ freedom of choice and human rights must be upheld at all times.
At TnC Care we believe in the social model of disability and our main aim is to look at how we can empower people to live the life of their choosing.
We work closely with the participant as an individual, their informal support networks such as families and friends and their formal supports such as allied health professionals and support workers. This allows us to help each person achieve their goals and to live as independently as possible.

Privacy and Confidentiality
TnC Care takes privacy and confidentiality seriously. All your personal information is protected under the Privacy Act 1988 and in line with our strict privacy procedures within our company. We also adhere to the NDIS Quality and Safeguard Commissions Standards that are set out in the National Disability Insurance Scheme Act 2013.
Any information that is provided to TnC Care staff will not be disclosed to any other party without the participants and/or their nominee’s consent. All documentation regarding your support from TnC Care will only be viewed by staff directly involved with your care and anyone else that you choose to have access to your information.
All documentation is kept strictly confidential within the company. No information, photographs, videos or any other materials will be posted on social media, flyers or advertising brochures without your written consent.
Vision and Mission
TnC Care strongly believe that every person should be treated as an individual and with the utmost respect. We understand that change is inevitable and continuous and as such, we adapt to each participant’s changing circumstances to ensure that you are being provided with priority care. We employ highly skilled and qualified staff to ensure that you are getting appropriate care at all times.
Our Dedicated Support Workers
Our Support Workers are from Australia as well as many staff who are from different cultural and linguistic backgrounds that have the passion and dedication for the role. The staff are all disability qualified support workers who have a range of different skills and attributes that they bring to the organisation.
All our staff at TnC Care have current Police Checks, Working with Children’s Checks, First Aid qualifications and CPR. Our staff have also all completed the infection control Covid-19 training as well as the Worker Orientation Module ‘Quality, Safety and You’.
We also provide all support workers with ongoing training to ensure they are continuously expanding their skill set and knowledge of the disability sector.
Our support workers provide a holistic person-centred approach in a variety of activities including but not limited to:
- Personal Care
- Household tasks including cleaning, laundry etc.
- De-cluttering
- Food preparation and cooking at home or delivered
- Shopping
- Home and Garden Maintenance
- Social and Community Participation
- Transport to activities and appointments
- Interpreter and Translation Services
- Short Term Accommodation/Respite
